Go to Nettskjema.no/en to get started and log on via Feide with your NIH username and password. You use the online form and there is no need to download any applications to your computer.

Important information regarding Sensitive Personal Data - Confidential and Strictly Confidential Data

NIH has rules for the processing and storage of various categories of data. See NIH classification guide for more information.

  • Black Data (Strictly Confidential) can cause significant damage if it goes astray. Black Data must be treated with extra care.
  • Red Data (Confidential) can cause major damage if strayed.
  • Yellow Data must have some protection.

Black Data cannot be collected via Nettskjema unless a link to TSD will be establised.

Red Data: Nettskjema can be used, but for the time being (November 2023) you must answer "no" to the initial question whether sensitive personal data will be collected. (If you answer "yes", you will be told that the form can only be used for "testing" or with a link to TSD. This is a technical/conceptual ambiguity that UiO is working to correct.)

For questions about this, contact Information Security Advisor Hans Olav Krogsæter or Privacy Advisor Kaja Stene.
